Job summary

TALENT Software Services is seeking a Registered Nurse for a field-based, hybrid role serving clients in the Phoenix area. The position requires 10% onsite travel and 90% remote work, with travel to client sites as needed.

You will complete nursing referrals, coordinate with interdisciplinary teams, and document services per policy. Applicants must be licensed RN with 4–5 years in home health, hospice, long-term care, or case management, including tube feeding experience.

Qualifications

• Licensed Registered Nurse (RN) in applicable jurisdiction.
• Minimum 4–5 years of RN experience in home health, hospice, long-term care, or case management.
