Ponos Care is seeking an RN Case Manager (TOC / Travel Infusion / RPM / LTSS) to support Medicaid populations with hybrid telephonic and in‑person care. You will manage a complex caseload, perform remote monitoring, and coordinate infusion therapies to improve outcomes.

The role requires a BSN, an active multistate RN license, CPR/BLS, and 2+ years in care management or infusion. Travel to patient homes may be involved; strong collaboration and commitment to equitable care are essential.

Formación

• Bachelor’s degree in nursing (BSN) required.
• Active multistate RN license in all states of practice.
• CPR/BLS certification required.
• Minimum 2+ years of clinical nursing experience in care management, infusion, or population health.
• Experience serving Medicaid or complex care populations preferred.

Responsabilidades

• Conduct comprehensive assessments via telephonic outreach or in‑home visits.
• Develop personalized care plans with the interdisciplinary team.
• Provide infusion therapies in members’ homes when needed.
• Coordinate services and referrals across the care continuum.
• Ensure documentation meets CMS, NCQA, and HIPAA standards.
