Job summary

CVS Health is seeking a Case Manager (RN) for a work-from-home role based in the Dallas Service Delivery Area, with up to 75% travel to in-home visits and occasional facility visits. The RN Case Manager will deliver high-quality care coordination through telephonic and face-to-face engagement to help members navigate their healthcare needs.

Responsibilities include assessments, care-plan development, coordinating services, education, documentation, and mentoring junior staff.

Qualifications

Texas RN license required
Minimum 3 years of clinical nursing experience
At least 1 year of case management experience
Must reside in Dallas Service Delivery Area

Responsibilities

Conduct telephonic and in-person assessments of members' medical and behavioral health needs
Develop and implement individualized care plans
Coordinate and monitor ongoing care plan activities
Collaborate in multidisciplinary care reviews to optimize health results
Identify and escalate quality of care concerns
Engage members using motivational interviewing and coaching techniques
Educate and empower members to participate in healthcare decisions
Secure appropriate services and resources through negotiation and coordination
Ensure accurate documentation adhering to regulatory standards
Serve as a preceptor and mentor to new nurses and clinical staff
Support onboarding by teaching policies and workflows
Foster a positive environment that promotes clinical excellence
