Comfort Keepers is seeking an experienced Registered Nurse (RN) Case Manager to perform client assessments, develop and monitor care plans, and provide supervision and caregiver support. The role is field-based with flexible, part-time/PRN or contract arrangements, requiring travel to client homes across the service area.

Ideal candidates will have 3+ years in home health or related settings, an active RN license, strong documentation skills in WellSky, and a passion for improving quality of

Competenze

• Active and unrestricted RN license.
• Minimum of three (3) years in Home Health, Hospice, Rehabilitation, Home Care, or PDN.
• Valid driver's license and reliable transportation.
• Ability to travel to client homes throughout the service area.
• Strong computer skills and ability to document electronically.

Mansioni

• Conduct intake calls and follow up with prospective clients and families.
• Perform initial in-home assessments and home safety evaluations.
• Develop, implement, and document individualized plans of care.
• Conduct periodic reassessments and update care plans as needed.
• Monitor client conditions and document changes appropriately.
