Job summary

CVS Health is hiring a Case Manager (RN) for a remote role requiring frequent local travel in the Dallas Service Delivery Area. You will conduct telephonic and in-home assessments and coordinate individualized care plans to improve member health outcomes.

As a licensed RN with case management experience, you will mentor staff, ensure HIPAA-compliant documentation, and support onboarding to promote clinical excellence within a flexible work-from-home environment.

Qualifications

• Active Registered Nurse license in Texas.
• 3+ years of clinical nursing experience.
• 1+ year of case management experience.
• Must reside in Dallas TX Service Delivery Area and be willing to travel up to 75%.
• Mileage reimbursed per policy.

Responsibilities

• Conduct telephonic and in-person assessments of members’ medical and behavioral health needs.
• Develop and implement individualized care plans aligned with clinical guidelines.
• Coordinate and monitor care plan activities to improve outcomes.
• Collaborate in multidisciplinary care reviews to optimize member health results.
• Educate and empower members for shared decision-making with providers.
• Document accurately per regulatory and company standards.
• Serve as a preceptor to new nurses and clinical staff.
