WelbeHealth is hiring a Home Care Coordinator in New Jersey to coordinate in-home care assessments and oversee personal care delivery. You will work with nursing, therapy, aides, social services, and physicians to schedule and manage care, ensuring high-quality participant outcomes.

The role requires an active RN license in NJ and at least 3 years of case management or home nursing experience with frail elderly individuals. A hybrid work model and a comprehensive benefits package are included.

Qualifications

• Active, unrestricted RN license in New Jersey is required.
• Bachelor's degree preferred.
• Minimum of three (3) years of case management or nursing experience in a clinical or home setting with a frail or elderly population.
• Nursing knowledge and training to treat frail, elderly participants and manage complex clinical conditions.

Responsabilités

• Coordinate incoming calls related to participants, physicians, and agency services regarding orders, questions, and referrals.
• Communicate with participants by phone and coordinate with nursing, therapy, aides, social services, and physicians for scheduling changes and updates.
• Collaborate with Home Care Services staff to track home care delivery and scheduling hours.
• Partner with Marketing to support enrollment of prospective participants into the program.
• Assist with staffing and scheduling activities, soliciting and incorporating manager input.
• Participate in end-of-life care, coordination, and support.
