Job summary

We offer a weekday schedule across Mercer, Middlesex, and Monmouth counties with rotating on-call support. Active NJ RN license and strong clinical judgment are required; home-care experience is preferred.

Qualifications

• Active New Jersey RN license required.
• Experience in home care or community-based nursing preferred.
• Strong clinical judgment and communication skills.

Responsibilities

• Conduct nurse assessments and build individualized care plans
• Provide oversight, coaching, and support to caregiving staff
• Coordinate with families and medical providers
• Adjust care plans as client needs change and follow through
• Able to perform the essential functions of the role with or without reasonable accommodation
