Job summary

Qualifications

• Five (5) years of professional nursing experience.
• Licensed to practice as a Registered Nurse in North Carolina.
• Graduation from a state-accredited school of professional nursing.

Responsibilities

• Assesses patient status of a specialized population and coordinates care with an interdisciplinary team.
• Provides patient education and participates in discharge planning and post‑hospitalization care coordination.
• Collects and analyzes data for performance improvement and benchmarks.
• Coordinates appointments with clinics and referral sources to ensure continuity of care.
• Documents care planning and communicates with providers and family.
