Qualifications

• Registered Nurse licensed in North Carolina (NC).
• Experience in case management, care coordination, or nursing leadership.
• Basic Life Support (BLS) for Healthcare Providers.

Responsibilities

• Education – identify learning needs, participate in education programs, assess readiness to learn, and tailor materials to patient populations.
• Evaluation of Care – identify outcomes, revise plan of care with team input, and communicate to ensure continuity of care.
• Implementation – provide care for patients with complex needs, perform bedside testing, follow policies, prioritize tasks, and involve appropriate resources.
• Leadership – develop and pursue goals, collaborate across units, participate in unit goals and performance improvement, promote cost-effective care, and mentor others.
• Patient Assessment – synthesize complex data to set priorities, assess discharge needs, and build family partnerships for advocacy.
• Planning – coordinate with patient/family to update the care plan, use multidisciplinary resources, and address cultural/ethnic issues in planning.
• Research – engage with journals and bring ideas to the team for consideration.
