Socket.dev is seeking a Full-Time Hybrid Case Manager (RN) to oversee and coordinate the delivery of resident care from pre-admission through post-admission at Montesano Health and Rehab. The role requires collaboration with families and healthcare teams to ensure high-quality, person-centered care.

The ideal candidate is a registered nurse with active licensure, strong clinical judgment, and experience in long-term care.

Qualifications

• RN program completed and eligible for licensure in the state
• Current, unencumbered RN license in the state
• Ability to make quick judgments and work independently
• Knowledge of nursing practices and long-term care regulations

Responsibilities

• Oversee and manage care delivery for residents from pre-admission through post-admission
• Coordinate services to meet residents' needs promptly and effectively
• Collaborate with healthcare providers, families, and interdisciplinary teams to adjust care plans
• Monitor resident progress and implement appropriate interventions
• Facilitate smooth transitions of care and ensure resources are in place
• Advocate for residents and families and provide education as needed
• Travel to support other buildings as part of the role
