Job summary

Socket.dev is seeking an RN Case Manager to support the Director of Nursing in facilitating safe and smooth discharges for all patients. The role is full-time, weekdays with occasional weekends and holidays, and requires coordinating discharge planning, paperwork, DME needs, and transportation for residents.

The RN will participate in team meetings, QA processes, and ensure confidentiality while educating staff on safety and care standards within a skilled nursing environment.

Qualifications

• Active RN license in the state where work is performed.
• Minimum 2+ years of discharge planning or care coordination experience in healthcare.

Responsibilities

• Communicate discharge planning effectively.
• Coordinate DME needs and transportation on day of discharge.
• Complete discharge paperwork and required documentation.
• Participate in interdisciplinary team meetings and QA review meetings.
• Maintain confidentiality of resident and facility information.
