Qualifications

As aFull -Time Hybrid Case Manager (RN) , you will oversee and manage the delivery of care for residents from pre-admission through post-admission. You will play a pivotal role in ensuring residents and their families receive appropriate care coordination and utilization of services to ensure the highest quality care.

Your key responsibilities include:

Must Have:

• Successfully graduated from an approved and certified RNprogram
• Current and unencumbered license to practice as anRNin the state
• Ability to make quick judgments and work independently
• Knowledge of nursing practices, procedures, terminology, laws, regulations, and guidelines that pertain to long-term care

Nice to Have:

• 2 years of experience as a RN
• Active patient or bedside care experience within the past three years
