Socket.dev is seeking a Full-Time Hybrid Case Manager (RN) to oversee and coordinate the delivery of resident care from pre-admission through post-admission at Montesano Health and Rehab. The ideal candidate is a registered nurse with active licensure, strong clinical judgment, and experience in long-term care.

Qualifications

Responsibilities

• Oversee and manage care delivery for residents from pre-admission through post-admission
• Coordinate services to meet residents' needs promptly and effectively
• Collaborate with healthcare providers, families, and interdisciplinary teams to adjust care plans
• Monitor resident progress and implement appropriate interventions
• Facilitate smooth transitions of care and ensure resources are in place
• Advocate for residents and families and provide education as needed
• Travel to support other buildings as part of the role

This is a Hybrid position meaning you will be traveling to Tacoma, Seattle and Puyallup to help support but your home base will be here at Montesano Health and Rehab.

As aFull-Time Hybrid Case Manager (RN), you will oversee and manage the delivery of care for residents from pre-admission through post-admission. You will play a pivotal role in ensuring residents and their families receive appropriate care coordination and utilization of services to ensure the highest quality care.

Your key responsibilities include:

• Managing care for residents throughout the care continuum, including pre-admission, admission, and post-admission stages.
• Coordinating services to ensure that residents' needs are met in a timely and effective manner.
• Collaborating with healthcare providers, families, and interdisciplinary teams to develop and adjust care plans based on resident needs.
• Monitoring the progress of residents and ensuring appropriate interventions are implemented when necessary.
• Facilitating smooth transitions of care, ensuring all services and resources are in place for each resident.
• Advocating for residents and families, ensuring their needs are met, and providing education as needed.
  
• Supporting/traveling to other buildings.

Must Have:

• Successfully graduated from an approved and certified RNprogram
• Current and unencumbered license to practice as anRNin the state
• Ability to make quick judgments and work independently
• Knowledge of nursing practices, procedures, terminology, laws, regulations, and guidelines that pertain to long-term care

Nice to Have:

• 2 years of experience as a RN
• Active patient or bedside care experience within the past three years
