Base Salary Range

$100,000.00 to $110,000.00 per year.

Job Details

Seeking experienced Bilingual Registered Nurses for a Hybrid and then Remote RN Care Manager role with a Managed Care Plan. Position involves 2 days in-office and 3 days remote, transitioning to fully remote after 3-4 months of in-office training in Manhattan, NY. Candidates must have NYS RN License, MLTC RN Care Management experience, and reside in downstate NY or NJ. Must be able to commute to Manhattan for training and attend meetings in-office as needed. Bilingual skills in Spanish and Chinese are in demand.

The RN Care Manager provides clinical oversight to ensure members receive safe, effective community care. Reports to the Supervisor of Care Management and develops, implements, and monitors care plans in collaboration with PCPs, caregivers, and team members, addressing medical, social, psychological, physical, and spiritual needs.

Qualifications:

  • Associate’s Degree in Nursing (preferred for experienced candidates)
  • NYS RN license required
  • CCM certification preferred
  • Minimum 1 year RN experience in MLTC or managed care required; consideration given to RNs with 2+ years in home care, acute care, or nursing homes

Salary ranges from $100,000 to $110,000, plus a $15,000 Sign-On Bonus and excellent benefits.
