Job summary

PrideGlobal is seeking an experienced Registered Nurse Care Manager for a hybrid position supporting older adults and individuals with complex healthcare needs in the Bronx. The role combines field visits with remote care coordination, offering flexibility while delivering high-quality, person-centered care; requires 3+ years RN experience, NY license, and ability to work in a hybrid schedule.

On-site training in Queens, salary $100,000–$130,000 with benefits including dental, vision, life

Qualifications

• Active, unrestricted New York RN license.
• Associate’s degree in nursing; BSN preferred.
• Minimum 3 years of RN experience.
• Experience in MLTC, home care, geriatrics, or case management preferred.
• Strong communication, organizational, assessment, and critical-thinking skills.
• Ability to work independently in a hybrid environment.

Responsibilities

• Conduct in-home assessments to evaluate members’ clinical, functional, and social needs
• Develop and manage individualized care plans in collaboration with physicians and interdisciplinary teams
• Coordinate care across home care, hospitals, rehabilitation facilities, and long-term care settings
• Complete approximately 6 home visits per week within the Bronx territory
• Monitor member progress and adjust care plans to support optimal outcomes
• Support safe discharge planning and help reduce hospital readmissions
• Communicate effectively with members, families, caregivers, and healthcare providers
• Maintain accurate and timely clinical documentation in accordance with applicable guidelines
• Advocate for members and connect them with appropriate healthcare and community resources
