Job summary

Children’s National Hospital is seeking a Registered Nurse Care Manager to provide comprehensive care coordination for HSCSN enrollees with high-complexity medical and behavioral needs. The role supports a caseload with focus on medical intervention and coordination across services.

The position requires an Associate’s RN (Bachelor’s preferred), 2 years’ care management experience, and strong communication skills. This hybrid role blends field visits with remote coordination within HSCSN network.

Qualifications

• RN licensure in DC or eligible to practice in DC.
• Minimum 2 years clinically related care management experience.
• Experience in discharge coordination, home health, utilization review or disease management.
• 1 year in a public/community health setting preferred.

Responsibilities

• Manages a caseload as directed by Care Management Leadership.
• Maintain compliance with contractual and care management requirements.
• Conduct outreach and face-to-face visits in homes, clinics, or other locations.
• Develop and update individualized care plans with PCP and multi-disciplinary team.
• Document enrollee findings and interventions in a timely manner.
• Facilitate care coordination and discharge planning activities as needed.
