Job summary

Qualifications

• Bachelor's degree in Nursing (BSN)
• Current NYS Registered Nurse (RN) licensure
• 5 plus years of health care experience
• Flexibility and ability to work in fast-paced, changing environment

Responsibilities

• Provide intensive care management for pediatric patients with complex medical needs.
• Develop individualized care plans and coordinate with a multidisciplinary team.
• Reduce hospital readmissions across the health system.
• Guide patients and families through primary and specialty care, screenings, treatment, and recovery.
• Follow up to ensure understanding of the care plan, resources, access to care, medication management, and home care needs.
