Job summary

Cityblock Health in Worcester, MA seeks an experienced RN Care Manager to support clinically complex members across programs. You will coordinate care plans, assess needs, and collaborate with the Cityblock Care Team and community partners to graduate members from pathways.

You will perform screenings (PHQ-9, GAD-7, AUDIT, DAST-10), conduct visits, manage medications, and document thoroughly. Hybrid work entails about 70% field and 30% remote with flexible scheduling.

Qualifications

• Graduate of an accredited school of nursing (R.N.).
• 3+ years of nursing experience with clinical judgment.
• Experience with behavioral health screening and care coordination.

Responsibilities

• Receives members from engagement and central teams, communicates program expectations.
• Conducts self-efficacy and condition-specific screeners, refers to behavioral health programs as needed.
• Performs in-person clinical exams and places members in programs of varying intensity.
• Prepares for and leads case conferences when necessary.
• Develops care plans with members and addresses social needs with Community Health Partner.
• Conducts regular clinical visits and follows program guidelines with timely follow-ups.
• Collaborates with care team to support a panel on health maintenance and chronic disease management.
• Performs medication reconciliation, administration, and education.
• Addresses quality gaps and ensures thorough chart documentation (ICD/CPT).
• Uses EHRs and scheduling platforms to track tasks and communicate with resources.
