Mass General Brigham Community Physicians, Inc. is seeking a Registered Nurse for case management and discharge planning within a hybrid work model. You will coordinate patient care, manage referrals, and engage with families to develop comprehensive care plans.

The role requires an active MA RN license and 2+ years of related experience. Schedule includes 40 hours weekly, with on-site and field work, and a starting pay range shown in the listing.

Formación

• Associate's Degree Nursing required.
• Bachelor's Degree Nursing preferred.
• Registered Nurse (RN) License for the State of Massachusetts required.
• 2+ years of case management, utilization review, or discharge planning experience required.

Responsabilidades

• Coordinate and support healthcare within the facility and referrals.
• Develop and coordinate community service plans with patients and families.
• Communicate with third-party payers for necessary authorization and reimbursement.
• Prepare patients and families for discharge and care transitions.
• Maintain accurate, detailed reports and records.
