Job summary

Village Care is seeking a Full Time RN- Care Manager in a hybrid role, open to residency within the New York Tri-State Area. You will apply nursing expertise to manage the care needs of a diverse patient population and coordinate services.

Salary ranges from $95,000 to $105,000 per year with a robust benefits package including PTO, holidays, health/dental/vision, retirement, and commuter benefits, plus involvement in discharge planning and care transitions with an interdisciplinary team.

Qualifications

• Valid NYS RN license.
• Minimum 3 years of experience in Managed Care, Home Care, or Community settings.
• Associate degree or higher is required.
• Bilingual proficiency in Korean, Bengali, Cantonese, Mandarin, Spanish, Russian, or Creole.

Responsibilities

• Care planning, coordination, and health assessments.
• Monitor quality and effectiveness of services and track progress toward goals.
• Oversee transitions and discharge planning from hospitals or nursing homes.
