Job summary

Astrana Health, Inc. seeks a Risk Adjustment Coordinator for a temporary assignment in the Inland Empire to retrieve medical records from providers and EMR systems. The role supports coders, audits, and sweeps with travel to provider offices as needed.

You will collect data, prepare reports, and coordinate meetings while upholding HIPAA and CMS risk adjustment standards. Hybrid work and 50% travel are required.

Qualifications

• Must be able to travel up to 50% of work time.
• High School diploma or GED required.
• Excellent written and oral communication skills, as well as strong interpersonal.
• Excellent organizational, decision-making, and multi-tasking skills.
• Proficient with Microsoft Office (Word, Excel, Power Point) and EZCAP.

Responsibilities

• Contact and collect medical records onsite from Provider’s offices or via EMR access documentation for coders/audits/sweeps retrieval.
• Collect and summarize meeting minutes from the meeting.
• Coordinate office activities (scheduling internal/external meetings and provider visits).
• Prepare reports for the meeting (weekly/monthly or quarterly, if needed).
• Distribute E-Fax and respond to emails and faxes for the team.
• Create/update monthly list for Active Members/Member Without Office Visit in IPA folder.
• Receive and organize data from external sources.
• Understand HIPAA principles and maintain confidentiality of patient health information.
• Understand Risk Adjustment CMS standards and communicate data needs.
• Attend provider and interdepartmental calls; participate in IPA and health plan meetings.
