Qualifications

Responsibilities

• Contact and collect medical records onsite or via EMR access documentation.
• Coordinate meetings and distribute minutes.
• Prepare weekly/monthly/quarterly reports.
• Distribute E-Fax and respond to team emails/faxes.
• Organize data from external sources; maintain HIPAA confidentiality.
• Support risk adjustment CMS principles and communicate with IPA/providers.
• Attend meetings and maintain positive relationships.
