Job summary

Alleghenies United Cerebral Palsy (AUCP) is seeking a Service Coordinator to partner with individuals and their support teams to develop, update, and manage personalized service plans. The role focuses on client-centered care and ensuring access to needed services and community resources.

This hybrid/remote position requires PA residency or willingness to travel for home visits and ongoing coordination with teams to achieve goals. Strong communication and compliance skills are essential.

Qualifications

• Bachelor's degree in social/human services or related field.
• At least three years of professional experience in case management for people with physical disabilities.
• Three years of experience working with individuals with Autism.
• Knowledge of Medicaid Waiver programs, LTSS, HCBS is strongly preferred.

Responsibilities

• Develop individualized service plans (ISP) with clients and teams.
• Regularly update service plans based on life changes and needs.
• Coordinate services with paid providers and community resources to ensure proper implementation.
• Maintain open communication with support teams and ensure timely documentation and billing compliance.
• Travel within the assigned region to meet with participants as required.
