Job summary

Strive Health is seeking a Nurse Practitioner to lead a defined patient panel across home, clinic, and telehealth settings. You will deliver longitudinal care for CHF, CKD/ESKD, and related conditions, balancing autonomy with collaboration in an interdisciplinary team.

Annual salary range is $112,000–$140,000 with a 10% target bonus; hybrid schedule with remote work portions and travel across the region. State licensure, board certification, and DEA eligibility are required.

Qualifications

• Master's degree in Nursing or related field is required.
• State licensure and board certification are mandatory.
• DEA license or eligibility within 90 days is required.
• BLS/CPR certification is required.
• Experience with EHR systems is expected.

Responsibilities

• Manage a defined patient panel with accountability for quality, cost and outcomes.
• Conduct history and physical exams; order and interpret tests; plan care.
• Care for CKD/ESKD and heart failure patients across settings, including home and telehealth.
• Document diagnoses for risk adjustment and care gaps; support care transitions.
• Collaborate with care teams and participate in rounds and meetings.
