Job summary

Strive Health in Winston-Salem, NC is seeking a Clinical Lead Nurse Practitioner to guide care for patients with CHF, CKD/ESKD and other chronic conditions, delivering longitudinal, home- and telehealth-based care with autonomy and interdisciplinary collaboration.

You will leverage analytics and technology to ensure high-quality outcomes, reduce hospitalizations, and support transitions. This role emphasizes a whole-person approach and ongoing professional development.

Qualifications

• Master's degree in Nursing or similar field.
• 2+ years experience as NP in cardiology or nephrology.
• 2+ years experience with EHR.
• Current state license.
• National Board Certification.
• Current DEA license or eligible within 90 days.
• BLS or CPR certification.
• Active driver's license and ability to travel.
• Strong computer skills and internet access.

Responsibilities

• Manage a defined patient panel with accountability for quality, cost and outcomes.
• Obtain patient history, perform exams, order tests, and plan care.
• Coordinate CKD/ESKD and heart failure care, including transitions.
• Document diagnoses for risk adjustment and care gaps.
• Present cases and support clinical rounds and interdisciplinary meetings.
• Provide care in homes, telehealth, clinics as needed.
• Maintain board certification and state licensure.
• Travel as required to meet patient needs.
