Job summary

Mercy Clinic Primary Care - Chesterfield Valley seeks a Primary Care Nurse to partner with Providers and the clinical team to deliver safe, timely, patient-centered care for an attributed panel.

The role includes on-site training for three months, then a 50/50 hybrid model. Schedule is Monday–Friday, 08:00–16:30, with no nights, weekends, or holidays. Licensure in LPN or RN and 2–3 years of experience preferred.

Qualifications

• LPN: Diploma in Practical Nursing, active LPN license, 2-3 years experience preferred.
• RN: AS in nursing, BSN or 4 years commensurate experience, active RN license, 2-3 years experience preferred.

Responsibilities

• Manage triage calls/messages for the clinic.
• Work with Providers and care team to drive quality outcomes.
• Move to hybrid work schedule after training; on-site training required.
