Job summary

Astrana Health, Inc. is seeking a Population Health Coordinator - ACM for our hybrid work environment in Orange, CA. This role will support ambulatory care operations and coordinate with ACM nurses to manage member care delivery.

Responsibilities include member outreach, eligibility checks, records requests, and participation in health plan meetings. Requires a high school diploma or GED and MS Office proficiency. Mandarin or Cantonese ability is a plus.

Qualifications

• High school diploma or GED required.
• Experience using Microsoft Word, Excel and Outlook.

Responsibilities

• Outreach to members/providers via phone or written communication using department templates.
• Assist ACM nurses with member assessment, eligibility verification, requesting medical records, creating cases, and documenting care activity.
• Participate in health plan meetings and trainings.
• Assist with health plan audit preparation.
• Ensure maximum utilization of resources.
• Exceed standard benchmarks with performance objectives.
• Provide ongoing mentorship to ACM Coordinator I and other team members as needed.
• Take the lead in delegated projects and assignments.
• Assist in developing training for new ACM coordinators.
• Perform other duties, projects and actions as assigned.
