Job summary

Sentara Health Plans is seeking a Care Coordinator for the Population Health Ambulatory Clinical Support Team in Charlottesville. This hybrid role involves 2-3 days per week in the office and on-site work at affiliate practices, with some remote days possible.

The role requires a Bachelor’s degree and licensure in Virginia as a Pharmacy Technician with a CPhT, or CMA/RMA/CCMA, or LPN, along with at least 1 year of Long-Term Care experience; ambulatory and clinical pharmacy experience and

Qualifications

• Bachelor's degree required.
• Licenses: Virginia Pharmacy Tech and CPhT, or CMA/RMA/CCMA, or LPN license.
• 1 year of Long-Term Care experience required; ambulatory and clinical pharmacy preferred; Medicaid/Health Plan experience preferred.

Responsibilities

• Perform case management and coordinate multidisciplinary services.
• Support adults and families with medical and environmental challenges, including end-of-life when appropriate.
• Collaborate with care teams to monitor patient progress and care transitions.
