Job summary

Sentara Health is seeking a Care Coordinator to join our Population Health Ambulatory Clinical Support Team in Charlottesville, VA. The role supports patients, families, and caregivers through case management and collaboration with a multidisciplinary network.

The hybrid position requires 2-3 days in the office and provides work-from-home days, with an 8:00 AM–5:00 PM schedule. A Bachelor’s degree and relevant licensure are required, with prior Long-Term Care experience preferred.

Qualifications

• Bachelor's Level Degree is required.
• 1 year of Long-Term Care experience is required.
• Ambulatory and Clinical Pharmacy experience preferred.
• 1 Year Medicaid or Health Plan experience preferred.

Responsibilities

• Provide case management services to patients, families, and designated caregivers.
• Develop, participate and monitor multidisciplinary collaboration of services to patients where appropriate.
• Assist adult patients and their families with personal and environmental difficulties related to medical conditions, including end-of-life care.

Sentara is hiring a Care Coordinator for our Population Health Ambulatory Clinical Support Team

Full Time (40 hours/week)

Shift

Days (Anticipated 8AM-5PM)

This Hybrid position requires 2-3 days per week in the office, in addition to days on site at affiliate practices, and allows for some work from home days.

The Care Coordinator performs a variety of casework duties and provides case management services to patients, families, and designated caregivers. Must develop, participate and monitor multidisciplinary collaboration of services to patients where appropriate. Assist adult patients and their families with personal and environmental difficulties associated with medical condition up to and including at time of terminal illnesses.

Education

• Bachelor's Level Degree (Required)

Certification/Licensure

• Virginia Pharmacy Tech License and CPhT License (Received through the NHA or PTCB)
• Certified Medical Assistant (CMA), or Registered Medical Assistant (RMA), or Certified Clinical Medical Assistant (CCMA)
• LPN Virginia state license

Ideally seeking a licensed Pharmacy Technician with a Certified Medical Assistant (CCMA) or LPN.

Experience

• 1-year of Long-Term Care experience (Required)
• Ambulatory and Clinical Pharmacy experience (Preferred)
• 1-Year Medicaid or Health Plan experience (Preferred)
