Job summary

Qualifications

• BSN required and RN license for Virginia.
• Ambulatory care coordination experience strongly preferred.
• Epic EMR experience highly valued.
• Strong communication and interpersonal skills essential.

Responsibilities

• Coordinate specialty appointments, imaging, and access to resources.
• Conduct hospital discharge follow-up and support safe transitions of care.
• Review diagnoses, medications, and next steps with patients.
• Navigate care needs across multiple specialties and services.
• Present cases to interdisciplinary teams and contribute to individualized care plans.
• Support population health and continuity-of-care initiatives.
• Build trusted relationships with diverse patients and communities.
• Document care in Epic and maintain an integrated patient journey view.
