Job summary

Sentara Health Plans in Central Virginia is seeking a Care Coordinator to perform case management for members with serious mental health and substance use disorders, including both face-to-face and telephonic assessments. Qualified candidates will hold a Health & Human Services related degree and have experience in mental health, including work

Qualifications

• Must possess a degree in Health & Human Services or related field.
• Experience in mental health and with children/adolescents is required/preferred.
• Strong communication and care coordination skills are essential.

Responsibilities

• Perform casework duties and provide case management.
• Develop and monitor multidisciplinary collaboration of services.
• Assist patients and families with personal and environmental difficulties related to medical conditions.
