Job summary

DaVita IKC is hiring an Engagement Specialist to onboard patients, schedule CHEs, and coordinate care with nurses and clinics. The role supports patient engagement, outbound outreach, and documentation across several clinics in a hybrid work model.

The position emphasizes strong communication, organization, and teamwork with care teams to ensure timely appointments and patient satisfaction. Flexible hours and a patient-centered environment are offered.

Qualifications

• High School Diploma or GED required; Advanced degree preferred
• 2+ years in customer service, patient-facing, or telephonic outreach roles preferred
• Healthcare experience preferred

Responsibilities

• Facilitate CHE completion in nephrology offices, dialysis clinics, or via telehealth
• Perform outbound calls to patients, caregivers, and clinics for CHE scheduling and reminders
• Welcome new patients to the program and collect new concerns for escalation
• Maintain accurate documentation and communicate updates to the care team
• Demonstrate interpersonal skills to build trust and engage patients
• Organize outreach by urgency and importance to maximize impact
• Coordinate care and referrals to assist with provider appointments
• Respond to inbound calls from patients, providers, and Village resources
