Job summary

DaVita Integrated Kidney Care is seeking a Care Coordinator to support patients with complex chronic conditions. You will coordinate referrals, access resources, and arrange provider appointments, while handling calls from patients and care teams.

A quiet home workspace and reliable internet are required when working from home. The role is full-time, with a hybrid schedule (clinic 2–4 days per week and remote otherwise).

Qualifications

• High school diploma or GED required.
• 2+ years in a professional office environment.
• Ability to handle confidential information with discretion and sound judgment.
• Strong verbal communication skills.
• Proficiency in Microsoft Office, Excel, and Outlook.
• Prior experience with Electronic Medical Records preferred.

Responsibilities

• Coordinate care for patients by securing access to resources and referrals and arranging provider appointments.
• Perform outbound and inbound calls from patients, providers, and Village resources.
• Complete non-clinical screenings and pathways under supervision and escalate as needed.
• Assist clinical team in moving patients through the continuum of care.
• Identify eligible resources and help members enroll in programs in collaboration with care nurses and clinics.
