Job summary

Alpine Physician Partners seeks a Patient Care Coordinator to support care coordination for Medicare patients in a hybrid setting near Denver. Qualifications

• Two years of experience as a Medical Assistant, Certification is required.
• Excellent knowledge of healthcare administration and patient care processes.
• Strong understanding of clinical practices and care coordination.
• Knowledge of Medical Terminology.
• Experience with Electronic Medical Records Management.

Responsibilities

• Coordinate care management activities with clinical teams.
• Act as a key information conduit between care teams and our systems.
• Engage patients in health improvement activities and educate on self-management to reduce ED visits.
• Participate in team huddles to review hospital census and population health data.
• Manage a panel of high-risk patients and coordinate with their PCP care team.
• Collaborate with patients, physicians, and staff to document progress and health goals.
