Zusammenfassung

Alpine Physician Partners is seeking a Patient Care Coordinator to support care management for Medicare patients within a primary care practice. You will work with clinicians to ensure seamless transitions of care, conduct post-discharge follow-ups, and educate patients on self-management to reduce unnecessary visits.

The role is hybrid with roughly 80% remote work and periodic in-clinic hours. A Medical Assistant background with CCMA CPCC certification preferred, and EMR experience is essential

Qualifikationen

• Two years of experience as a Medical Assistant.
• CCMA certification preferred with experience in primary care setting or care management a plus.
• Excellent knowledge of healthcare administration and patient care processes.
• Strong understanding of clinical practices and care coordination.
• Knowledge of Medical Terminology.
• Experience with Electronic Medical Records Management.

Aufgaben

• Coordinate care management activities with clinical teams, including pre-visit planning and post-discharge summaries.
• Act as a conduit of information between healthcare entities and care teams and systems.
• Engage patients in health improvement activities and educate them on self-management to avoid ED visits and hospitalizations.
• Support team huddles to review population health data and close gaps in care for high-risk patients.
• Actively manage a panel of high-risk patients and coordinate with their PCP care team.
• Collaborate with patients and care teams to document progress toward health goals.
