Job summary

Colorado Physician Partners is seeking a Patient Care Coordinator to support Medicare patients within a primary care practice. The role blends remote work (about 80%) with periodic in-clinic hours in the Denver area, focusing on care transitions and patient engagement.

You will collaborate with physicians, nurses, and care teams to ensure timely follow-ups, accurate documentation, and adherence to HIPAA. Requires MA certification and 2+ years of experience; bilingual abilities are a plus.

Qualifications

• Two years of experience as a Medical Assistant required.
• Certification as Medical Assistant required.
• Knowledge of healthcare administration and patient care processes.
• Experience with Electronic Medical Records (EMR) management.
• Bilingual (English/Spanish) preferred.

Responsibilities

• Coordinate care management activities with clinical teams.
• Facilitate transitions of care and post-discharge follow-ups.
• Act as information conduit between healthcare entities and care teams.
• Support high‑risk patients with gap closure and care planning.
• Maintain HIPAA-compliant documentation and patient confidentiality.
