Job summary

Alpine Physician Partners Colorado is seeking a Patient Care Coordinator to support care coordination and engagement for Medicare patients in a hybrid clinic setting in Denver, CO. The role collaborates with clinicians, ensures seamless transitions of care, and promotes patient self-management to improve outcomes.

The position requires 2 years as a Medical Assistant, CCMA preferred, and experience with EMR systems; bilingual in English/Spanish is a plus.

Qualifications

• Two years of experience as a Medical Assistant.
• CCMA certification preferred with experience in primary care setting or care management a plus.
• Excellent knowledge of healthcare administration and patient care processes.
• Strong understanding of clinical practices and care coordination.
• Knowledge of Medical Terminology.
• Experience with Electronic Medical Records Management.

Responsibilities

• Coordinate care management activities with clinical teams to plan pre-visit care and post-discharge steps.
• Act as a conduit of information between healthcare entities and care teams.
• Engage patients in health improvement activities and educate them on self-management to reduce ED visits.
• Support team huddles to review population health data and close gaps in care.
• Manage a panel of high-risk patients and coordinate with PCP care teams.
