Job summary

Bookmark Medical is hiring a Care Coordinator for a hybrid role combining remote and in-person work. You will partner with clinicians to close care gaps, identify patients for Annual Wellness Visits, and support quality care across the care team.

The role emphasizes patient advocacy, efficient resource use, and strong communication with patients and care teams. Minimum qualifications include CPR/BLS and 2–3 years as a Medical Assistant or LPN.

Qualifications

• Proficient in EMR and documentation practices.
• Knowledge of primary care concepts, care coordination, and health promotion.

Responsibilities

• Interview and patient intake to gather medical information.
• Coordinate and communicate with the health team to plan and execute the care plan.
• Perform clinical tasks and procedures as needed in the office.
• Identify patients for wellness visits and close care gaps.
• Document care accurately in the EMR and support quality measures.
