Job summary

Align Services in Wyoming is hiring part-time, hybrid Disability and Aging Case Managers. Two positions, caseloads involving intake, funding, and coordination of services for individuals eligible for waiver programs.

• Experience in coordinating services for aging populations and individuals with disabilities.
• Knowledge of disabilities regulations and ability to assess needs and advocate accommodations.
• Strong communication and collaboration skills with clients, families, employers and partners.

Responsibilities

• Assess clients' needs and develop case plans from intake through services.
• Coordinate services and monitor progress and documentation for waiver-eligible individuals.
• Educate clients on available programs and ensure compliance with policies and regulations.
