Hybrid role at NKC Health Hospital in North Kansas City, MO, with 40 hours/week. Requires Missouri RN license, 3+ years acute care experience, and discharge planning proficiency, plus EMR documentation and driving to facilities.

Qualifications

• Current, unrestricted RN license in Missouri.
• Must live within 30–60 minutes driving distance to NKC Health Hospital.
• Minimum 3 years of recent acute care nursing experience.
• Experience with discharge planning or care coordination in hospital or health plan settings.
• Experience using Microsoft Office and electronic medical records.
• Valid driver’s license and vehicle insurance.

Responsibilities

• Conduct comprehensive face-to-face assessments of newly admitted members at acute care facilities.
• Educate members and families on discharge planning and post-discharge follow-up.
• Identify gaps in care and barriers to discharge; collaborate on solutions.
• Collaborate with providers and care teams to ensure timely, coordinated care.
• Provide education on diagnoses, medications and preventive measures.
• Complete all required documentation accurately and promptly.
• Maintain infection control and patient privacy standards.
