Job summary

Humana is seeking an Onsite Care Coordination Nurse to support members during critical moments, providing face-to-face assessments and leading care coordination in acute care settings.

The role blends in-person work at assigned facilities with the flexibility to complete coordination and documentation remotely in a hybrid schedule. 40 hours weekly; Missouri RN license required with 3+ years acute care experience.

Qualifications

• Current, unrestricted RN license in Missouri.
• Minimum of 3 years acute care nursing experience.
• Experience with discharge planning or care coordination.
• Experience with MS Office and EMRs.
• Valid driver’s license and vehicle insurance.

Responsibilities

• Conduct face-to-face assessments for newly admitted members in acute care facilities.
• Lead discharge planning from admission through post-discharge.
• Identify gaps in care and develop strategies to address them.
• Collaborate with providers and care teams for timely, coordinated care.
• Educate on diagnoses, medications, and preventive measures.
• Complete documentation accurately and promptly.
• Maintain infection control and patient privacy standards.
