Job summary

Humana in North Kansas City, MO is seeking an Onsite Care Coordination Nurse to support members during transitions from hospital to home. You will conduct face-to-face assessments and guide discharge planning in collaboration with providers and care teams.

This hybrid role blends in-person work at an assigned facility with remote documentation. The ideal candidate is an experienced acute care nurse with strong communication skills and a commitment to whole-person care.

Qualifications

• Current, unrestricted RN license in Missouri.
• Located within 30-60 minute driving distance to NKC Health Hospital from home base.
• Minimum of 3 years recent acute care nursing experience.
• Experience with discharge planning or care coordination in hospital or health plan settings.
• Experience in Microsoft Office applications (Word, Outlook, Excel) and electronic medical records.
• Valid driver’s license, dependable automobile, and proof of personal vehicle liability insurance.

Responsibilities

• Conduct comprehensive face-to-face assessments of newly admitted members at assigned acute care facilities.
• Lead discharge planning from admission through post-discharge, including follow-up visits with primary care providers.
• Identify gaps in care and barriers to discharge; develop strategies to address these issues.
• Collaborate with attending providers, facility case managers, and other healthcare professionals for coordinated care.
• Provide education on diagnoses, medications, and preventive measures to reduce readmission risk.
• Complete all required documentation accurately and promptly.
