Job summary

Humana is seeking an Onsite Care Coordination Nurse to support members during transitions from hospital to home. You will perform face-to-face assessments, coordinate discharge planning, and collaborate with care teams to improve outcomes.

The role combines in-person facility work with remote documentation, within a 30–60 minute commute of NKC Health Hospital. The ideal candidate has a Missouri RN license, 3+ years acute care experience, discharge planning expertise, and proficiency in Microsoft

Qualifications

• Current, unrestricted Registered Nurse (RN) license in Missouri.
• Must reside within 30-60 minute driving distance to NKC Health Hospital-2800 Clay Edwards Dr., North Kansas City, MO.
• Minimum of 3 years of recent acute care nursing experience.
• Experience with discharge planning or care coordination in hospital or health plan settings.
• Experience with Microsoft Office and electronic medical records.
• Valid driver’s license, dependable automobile and proof of personal vehicle liability insurance.

Responsibilities

• Conduct comprehensive, face-to-face assessments of newly admitted members to evaluate physical, psychosocial, and health needs.
• Lead discharge planning from admission through post-discharge and facilitate follow-up visits with primary care providers.
• Identify gaps in care, safety concerns, and barriers to discharge; develop and implement mitigation strategies.
• Collaborate with attending providers, facility case managers, and other healthcare professionals to ensure timely, coordinated care.
• Provide education on diagnoses, medications, and preventive measures to promote wellness and reduce readmission risk.
• Complete all required documentation accurately and promptly.
• Maintain adherence to infection control, safety, and patient privacy standards.
