Job summary

Humana seeks an Onsite Care Coordination Nurse to support members during critical care transitions. You will conduct face-to-face assessments at assigned hospitals, coordinate discharge planning, and collaborate with care teams to ensure safe, timely moves from hospital to home.

The role blends in-person engagement with remote documentation, requires an Indiana RN license and 3+ years acute care experience, and offers a hybrid schedule with a 40-hour week.

Qualifications

• Active RN license in Indiana
• Minimum 3 years of acute care nursing experience
• Experience with discharge planning or care coordination in hospital or health plan settings
• Proficiency with Microsoft Office applications and electronic medical records
• Valid driver’s license, reliable vehicle, and insurance

Responsibilities

• Conduct comprehensive face-to-face assessments of newly admitted members at acute care facilities
• Lead discharge planning from admission through post-discharge
• Identify gaps in care and barriers to discharge; implement strategies
• Collaborate with providers, facility case managers and care teams for coordinated care
• Educate members on diagnoses, medications and preventive measures
• Complete documentation accurately and promptly
• Maintain infection control, safety and patient privacy standards
