Job summary

Humana is seeking an Onsite Care Coordination Nurse in South Bend, IN, to provide face-to-face assessments and lead care coordination for newly admitted members, ensuring safe transitions from hospital to home. The role blends in-person facility work with remote coordination and documentation.

Qualified candidates will have an Indiana RN license, at least 3 years of acute care experience, and strong discharge planning or care coordination background. A BSN and EMR proficiency are preferred.

• Current, unrestricted RN license in Indiana.

Responsibilities

• Conduct face-to-face assessments of newly admitted members at acute care facilities.
• Assist with discharge planning from admission through post-discharge.
• Identify gaps in care and develop strategies to address them.
• Collaborate with attending providers, facility case managers, and other health professionals.
• Educate on diagnoses, medications, and preventive measures to reduce readmission risk.
• Complete required documentation accurately and promptly.
• Maintain infection control, safety, and patient privacy standards.
