Job summary

Humana Inc. seeks an Onsite Care Coordination Nurse to support members during hospital-to-home transitions. You will conduct face-to-face assessments at Deaconess Gateway Hospital in Newburgh, IN, coordinating discharge planning with care teams.

This hybrid role requires 3+ years acute care experience, Indiana RN license, and proficiency in EMR and MS Office. Some travel for training may be required; comprehensive benefits are included.

Qualifications

• Active, unrestricted Indiana RN license.
• Minimum of 3 years of recent acute care nursing experience.
• Experience with discharge planning or care coordination.
• Proficient in Microsoft Office and electronic medical records.
• Valid driver's license and access to personal vehicle.

Responsibilities

• Conduct comprehensive face-to-face assessments for newly admitted members at acute care facilities.
• Educate members and families on discharge planning and follow-up with providers.
• Identify gaps in care and barriers to discharge; coordinate with care teams.
• Provide diagnoses, medication education, and preventive guidance.
• Document accurately; maintain infection control and patient privacy standards.
