You will perform face-to-face assessments, lead discharge planning, and collaborate with care teams to ensure safe transitions from hospital to home, aiming to reduce readmissions and improve outcomes.

Qualifications

• Current, unrestricted RN license in Indiana.
• Must reside within a 30–60 minute drive to Memorial Hospital – 615 N Michigan St, South Bend.
• Minimum 3 years of recent acute care nursing experience.
• Experience with discharge planning or care coordination in hospital or health plan settings.
• Proficiency with Microsoft Office and electronic medical records.

Responsibilities

• Conduct comprehensive, face-to-face assessments of newly admitted members at acute care facilities.
• Lead discharge planning from admission through post-discharge; facilitate follow-up with primary care providers.
• Identify gaps in care, safety concerns, and barriers to discharge; develop improvement strategies.
• Collaborate with attending providers, case managers, and care teams for coordinated care.
• Educate on diagnoses, medications, and preventive measures to reduce readmission risk.
• Complete documentation accurately and promptly; maintain infection control and patient privacy.
