Job summary

Humana is seeking an Onsite Care Coordination Nurse in South Bend, IN at Memorial Hospital (615 N Michigan St). You will conduct face-to-face assessments, lead care coordination for newly admitted members, and support discharge planning to ensure safe transitions from hospital to home.

Hybrid work combines facility presence with remote coordination. Requires Indiana RN license, 3 years acute care, and experience with discharge planning.

Qualifications

• Current, unrestricted Registered Nurse license in Indiana.
• Minimum 3 years recent acute care nursing experience.
• Experience with discharge planning or care coordination in hospital or health plan settings.

Responsibilities

• Conduct comprehensive face-to-face assessments for newly admitted members at assigned acute care facilities.
• Support and educate members and families on discharge planning from admission through post-discharge.
• Identify gaps in care, safety concerns, and barriers to discharge; develop and implement strategies.
