Qualifikationen

• BSN required and active in Texas.
• Minimum of two years oncology nursing experience (ambulatory/acute).
• OCN certification preferred.
• Strong telephone triage, care coordination, patient education, and interdisciplinary collaboration.

Aufgaben

• Facilitate entry of patients into the cancer program and serve as the point of contact for PCPs and referring providers.
• Provide individualized assistance to Medical Oncology patients, families, and caregivers in coordinating and communicating the plan of care.
• Initiate and document patient teaching related to diagnosis, treatment plans, symptom management, medications, and supportive care services.
• Perform telephone triage and assess patient concerns to ensure timely intervention and follow-up.
• Follow up on recommendations from Harris Health System's Ask My Nurse service.
• Implement risk assessments and healthcare pathways for oncology patients.
• Collaborate with the interdisciplinary team to coordinate and maintain individualized patient care plans.
• Develop collaborative relationships with all physicians in the UT MD Anderson Oncology Program at LBJ Hospital.
• Serve as a liaison between oncologists and patients' referring providers to support continuity of care.
• Provide access to community resources and connect patients with support services.
• Assist with tracking program metrics and quality indicators.
• Serve as a liaison between community personnel and the interdisciplinary team at LBJ Hospital.
