Job summary

Stanford Health Care in Palo Alto seeks an Oncology Nurse Navigator (RN) to coordinate cancer care from diagnosis to survivorship. You will collaborate with physicians, care coordinators, social workers, pharmacists, and other specialists to optimize patient outcomes.

Qualifications

• Licensed nurse with cancer clinical care coordination experience.
• Works with physicians, care coordinators, social workers and other team members to coordinate care.
• Provides navigation from diagnosis to survivorship and supports shared decision-making.

Responsibilities

• Ensures clinical care meets NCCN guidelines and pathways.
• Assesses patient education needs and information about trials and options.
• Addresses barriers to care in collaboration with social work and nutrition.
• Facilitates communication within cancer care team and provides nurse navigation.
• Focuses on referrals and second opinions.
